Welcome and thank you for taking time to complete this important report. The Annual Activity Report is where we confirm your participation in our membership requirements and identify other ways we can provide assistance to you and your firm.

We ask that you complete this report no later than February 7, 2020. Please let us know if you need extra time or assistance in any way. Completion of your firm's Annual Activity Report is a requirement of your membership in Selected.

Please note: In an effort to maintain confidentiality of company information, only those individuals identified as "key contact" for the funeral home will have access to this online Activity Report. To authorize another staff member to have access, the key contact will need to send an email to pam@selectedfuneralhomes.org or call us at 800-323-4219 to let us know.
